MY FIRST DAY OF PHYSICAL THERAPY
We want your first visit at ProSport to be a great experience and as smooth as possible. To prepare, please download the appropriate body-part paperwork below, complete the forms and bring them in on your first day. To learn more about

Please arrive to our office 10-15 minutes before your first scheduled appointment. Be sure to bring your paperwork, a photo ID, your insurance card and the prescription for physical therapy if you have one. Please wear comfortable attire so your therapist can access your injured area. We do have space to change and a shower if needed. If the patient is a minor, a parent or guardian must accompany them on their first visit. We are looking forward to working with you!
FREQUENTLY ASKED QUESTIONS
Q. Do I need a prescription/referral from a doctor to come in?
A. As of January 1, 2014, physical therapists in California can treat patients directly without a physicians prescription for up to 12 visits over 45 calendar days. Some insurance programs have been slow to adopt this change however. To be sure, contact your insurance provider or call our front desk at 949.250.1112.
Q. What insurance plans do you accept?
A. We accept most all PPO plans, Workers Compensation and Medicare. Unfortunately we do not accept Covered California plans or liens. Please click here to confirm that your specific plan is covered.
Q. What do I need to bring to my initial visit?
A. Please bring your insurance card, identification, referral/prescription if available and your completed paperwork which can be found above on our website or e-mailed/faxed to you when scheduling your evaluation.
Q. What should I wear to physical therapy?
A. We recommend that patients wear loose fitting or athletic-type clothing that allows skin access to the body part being treated. Tennis or walking shoes are preferred.
Q. How long will my appointment last?
A. Most appointments take anywhere from 60-90 minutes. If you are limited on time, please inform your therapist when you arrive and we can modify your treatment/exercise program to accommodate your needs.
Q. Will I work with my therapist the entire time?
A. All manual therapy will be performed by your physical therapist. We also have physical therapy aides who are extensively trained to assist with patient's exercise programs. Those programs are created by your physical therapist specific to your needs/goals.
Q. Is there a cancellation policy?
